Students are often required to submit forms for various reasons.
Here is an index of forms likely to be of interest to undergraduates
in the Department of Computer Science.
- College Petition
This form is used to request any changes to departmental or College of Engineering and Applied Science rules.
- Change of Major Form
This form is used by students in the College of Engineering and Applied Science
to change their major to another department within the College,
to add or delete a second major within the College,
or to add or delete a minor.
- Incomplete Grade Form
This form is used by a student and his/her instructor to request assignment of
an I (Incomplete) grade when, for documented reasons beyond the
student's control, the student has been unable to complete course requirements.
- Minor Completion Form
This form is used by a students in a college or school at CU-Boulder
outside of the College of Engineering and Applied Science to request
granting of a minor degree offered by the College.
- BS Degree Audit Forms
Degree audit forms are available to assist students in evaluating their
progress towards degree requirements.
- Independent Study Agreement
Students wishing to take an independent study course must complete this form.
- Senior Thesis Forms
There are a number of forms used in the management of a
Senior Thesis:
- Concurrent BS/MS Degree Forms
There are several forms used in the management of a
Concurrent BS/MS Degree:
- Computer Science Course Transfer Evaluation Request
This online form is used to request an evaluation of computer science
courses taken elsewhere to determine how the courses apply towards a Computer
Science degree.
- Request to Waive CSCI 1300 Degree Requirement
An experienced programmer pursuing a BS in Computer Science may be able to
successfully petition to skip the introductory computer science course
CSCI 1300 and go directly into CSCI 2270.
- Course Schedule Approval Form
This form is used by a student while on academic probation and when
returning from academic suspension to have their schedule of courses approved.
