Students are often required to submit forms for various reasons.
Here is an index of forms likely to be of interest to graduate students
in the Department of Computer Science.
There are general forms that are applicable to all graduate students,
as well as forms specific to students seeking MS, ME and PhD degrees.

Incoming MS students must develop a plan of study upon entering the program.
- Master's Examination or Project Report
This form is used to indicate the committee's evaluation of a
Master's Comprehensive Examination or Master of Engineering Project.
Master's comprehensive/final exam, thesis or project defense must be
scheduled with the Graduate School at least two weeks before the exam is held
by submitting this form.
- Master's Degree Plan Approval
This form is used for students completing the non-thesis degree.

Incoming PhD students must develop a plan of study upon entering the program.
- Area Exam Report
This form indicates successful completion of the PhD Area Exam.
- Annual Report of Progress
This form is submitted annually to indicate a student's academic progress.
- Doctoral Examination Report
This form is used to indicate the committee's evaluation of a
Doctoral Examination.
Doctoral Comprehensive Examinations and Dissertation Defense must be
scheduled with the Graduate School at least two weeks in advance by
submitting this form.
- Dissertation Defense Announcement
This form is used to announce a scheduled Dissertation Defense.
