Forms for Graduate Students

Students are often required to submit forms for various reasons. Here is an index of forms likely to be of interest to graduate students in the Department of Computer Science. There are general forms that are applicable to all graduate students, as well as forms specific to students seeking MS, ME and PhD degrees.

• General Graduate Forms
  • Candidacy Application for an Advanced Degree

    This form is used to apply for the granting of an advanced degree.

  • Request for Transfer of Credit

    This form is used to request a transfer of credit from another institution.

  • Request for Transfer of Credit within CU System

    This form is used to request a transfer of credit from another campus in the CU System.

  • Independent Study Agreement

    Students wishing to take an independent study course must complete this form.

  • Graduate School Student Travel Application

    The Graduate School offers Student Travel Grants which provide partial funding for graduate students to present research findings at meetings or conferences outside Colorado.

  • Application for Department Conference Travel Stipend

    This form is used to apply for Departmental support for graduate student presentations at technical conferences. The Graduate Committee requires applicants to obtain a statement from an appropriate CU faculty member (email is acceptable) that attests to the quality of the conference (e.g., top tier, acceptance rate, stature) and whether the student will be presenting a paper. Applicants are required to apply for the Graduate School Student Travel Grant.

• Concurrent BS/MS Degree Forms
  • Application for Admission to the Concurrent Bachelor's/Master's Degree Program

    This form is used to apply to the Concurrent BS/MS Degree Program.

  • Computer Science BS/MS Application for Admission

    In addition to the Graduate School form above, this form is used to apply to the Concurrent BS/MS Degree Program in the Department of Computer Science.

  • Certificate of Completion of the Requirements for the Bachelor's Degree

    This form is used to certify that a student pursuing a concurrent BS/MS degree has completed all requirements for the BS degree.

  • Mid-Program Review and Recommendation for Advancement to Graduate Status

    All concurrent BS/MS degree students must undergo a mid-program review to determine eligibility to advance to the graduate portion of the program.

  • Supplement to the Application for Admission to Candidacy

    Concurrent BS/MS degree students must submit this form to be admitted to degree candidacy.

• MS Degree Forms
  • MS Plan of Study

    Incoming MS students must develop a plan of study upon entering the program.

  • Master's Examination or Project Report

    This form is used to indicate the committee's evaluation of a Master's Comprehensive Examination or Master of Engineering Project. Master's comprehensive/final exam, thesis or project defense must be scheduled with the Graduate School at least two weeks before the exam is held by submitting this form.

  • Master's Degree Plan Approval

    This form is used for students completing the non-thesis degree.

• ME Degree Forms
  • ME Plan of Study

    Incoming ME students must develop a plan of study upon entering the program.

  • Master's Examination or Project Report

    This form is used to indicate the committee's evaluation of a Master's Comprehensive Examination or Master of Engineering Project.

  • Master's Degree Plan Approval

    This form is used for students completing the non-thesis degree.

• PhD Degree Forms
  • PhD Plan of Study

    Incoming PhD students must develop a plan of study upon entering the program.

  • Area Exam Report

    This form indicates successful completion of the PhD Area Exam.

  • Annual Report of Progress

    This form is submitted annually to indicate a student's academic progress.

  • Doctoral Examination Report

    This form is used to indicate the committee's evaluation of a Doctoral Examination. Doctoral Comprehensive Examinations and Dissertation Defense must be scheduled with the Graduate School at least two weeks in advance by submitting this form.

  • Dissertation Defense Announcement

    This form is used to announce a scheduled Dissertation Defense.
