Rarely does a set of 5 pages go by without such an instance, and I don't feel like listing them all here. Moreover, the webpage that was supposed to contain all the support materials doesn't, and the materials that ARE there are riddled with formatting errors. I was planning on saving up all the information that I find and sending it to Prentice-Hall and the author at the end of the semester, along with a selection of the frustrated 1am email messages that I get from my students every day.
